Basically there's a price (per month) per FORM and a price (per month) per user.
Forms are graded into one of three categories according to complexity of features.
Simple forms are £8 per month ( each) Standard forms are £12 per month (each) and Complex forms are £15 per month (each) plus VAT in all cases.
The cost per user per month is £5 plus VAT
The increasing form price reflects the additional design and setup time required where lookups and (especially) updates to other existing databases is part of your requirements. these costs are usually greatly offset when you consider the cost of manually transcribing the information from paper forms into your database
There may be circumstances where.there may be an additional cost if the setup time is expected to be exceptionally long - we will ALWAYS inform you in writing before agreeing the monthly cost and this will also be recorded in a formal contract for the protection of both parties.
